Kenneth Curry, 32, was last seen in the village of Westhampton Beach, Seafield Lane, at about 9pm on Monday 10 July.
According to Southampton Township Police, he suffered from a traumatic brain injury, bipolar disorder and is believed to be suicidal.
Curry is 6 feet tall, 165 pounds, with red hair and blue eyes.
At the time of his disappearance, he was wearing a thin gold bracelet and a thin gold necklace. He has a half-sleeve colored tattoo on his left arm and a leprechaun tattoo on his leg.
Police believe he may be in Nakajima.
