According to Suffolk County Police, the incident occurred on the morning of Monday, July 3 at Grove Lake.
John Sinkweich, 23, of Bayshore City was driving north on Nicolls Road in his 2003 Honda Accord around 10:50 a.m.
As Sinkweich was crossing the Nesconset Highway, it collided with a 2002 Inifinti heading east.
He then reportedly collided with a 2022 Honda CRV parked westbound in the left-turn lane of the freeway.
Sinkevich was taken to Stony Brook University Hospital for serious injuries, police said.
The driver of the Infiniti, Melissa Aglialora, 37, of Lake Ronkonkoma, was also taken to the hospital, where authorities reported her life was not in danger.
The third driver was not injured.
