It happened at around 11.50am on Sunday, June 18 in Manorville.
Suffolk Police said a 38-year-old Shirley woman was driving a 2022 BMW southbound on Fording River Road when she attempted a left turn onto South Street.
The BMW then collided with a 2020 Kawasaki motorcycle traveling north through the intersection.
Motorcycle driver Delio Dicuffa, 50, and his passenger, his wife, Loraine Dicuffa, 49, were all ejected from the vehicle, police added.
Loraine Dicuffa was taken by ambulance to Peconic Bay Medical Center in Riverhead, where she was pronounced dead.
Delio Dicuffa was taken by Suffolk Police helicopter to Stony Brook University Hospital where he was treated for serious bodily injuries.
The driver of the BMW was not injured.
