Arturo Rabassa, 93, was last seen on Wednesday, May 24 at 8:30 a.m. on Route 112 in the hamlet of Port Jefferson Station.
He suffers from dementia and may need medical attention, according to Suffolk Police.
Labasa is described as Caucasian, 5-foot-1, 150 pounds, with a bald head and blue eyes.
He was wearing a black hat, black sweater, gray pants and black shoes when he went missing.
