Charles Ford, 58, was reported missing and may require medical attention, according to the New York State Missing Persons Clearinghouse.
Ford was last seen on Hempstead Street at 11am on Sunday, May 7.
He is described as a black male, 5 feet 11 inches tall and 225 pounds. He is bald and has brown eyes.
Ford was last seen wearing a blue jacket with red stripes, a burgundy shirt and blue sneakers.
Authorities said he had gold teeth and a scar from head to jaw.
His destination is unknown.
Detectives are urging anyone who may know Ford’s whereabouts to contact the Nassau County Police Department’s Missing Persons Unit at 516-573-7347 or 911.
