According to Suffolk Police, David Smith was last seen leaving his Mastic Beach home on Cedar Road East at around 5pm on Monday, May 1.
The family reported the teenager missing on Friday, May 5.
Smith is described as a black, Hispanic male, 6-foot-1 and 220 pounds. He has brown curly hair and brown eyes and was last seen wearing a black hooded sweatshirt and blue jeans.
