Jazara Clark, 15, was last seen on Townhouse Drive, Colum, at 11pm on Sunday, June 11, Suffolk Police reported.
She is considered suicidal and may require medical attention.
Clark is described as a black female standing 5 feet 8 inches tall and weighing 170 pounds. She has black hair and brown eyes.
Clarke was last seen wearing a red shirt, blue shorts and pink slippers and was last seen walking, authorities said.
